Job Type: Full-time 37 hours per week

Salary: £20,195.00 /year

We are recruiting for a “Health and Wellbeing Community Officer. This role is part-funded through the Building Better Opportunities project by the European Social Fund and Big Lottery Fund.

If you can demonstrate excellent organisation and communication skills and are able to process information accurately and efficiently we would welcome your application. The successful candidate will be able to demonstrate outstanding attention to detail in planning and delivery of a variety of tailored engagement activities in community settings and across Ideal for All operational sites (in line with funder and project requirements). The ideal candidate is likely to have relevant experience of supporting contracts or projects and experience of working on ESF projects is desirable.

Essential:

  • Evidence of relevant education and/or qualifications level 2 above in Horticulture.
  • Experience of healthy lifestyle behaviour/nutrition awareness.
  • Experience of basic cooking skills with children, families, young people and vulnerable and disabled groups
  • Full UK Driving Licence and access to own fully insured vehicle for business use
  • Basic knowledge of supporting people in mental health and wellbeing settings

    Job Summary
  • To plan and deliver a variety of tailored engagement activities in community settings and across Ideal for All operational sites in line with varied project requirements and funded programmes.
  • To undertake all duties necessary to assist the Growing Opportunities employees and wider community in their efforts to help develop an interest in agriculture and horticultural, nutrition and health/mental health and wellbeing activity.
  • To support and deliver the transition of engaged participants onto time limited programmes at our sites or partner venues as part of integrated health/wellbeing/employment and skills programmes.
  • To undertake necessary delegated admin duties including recording and uploading of documentation in line with ESF procedures onto a MIS (management information system); initial engagements, action plans and progress reviews, induction paperwork/health &safety recording, SMART person-centred target measures, health/mental health & wellbeing and employment indicators, any other project specific based forms and measures/ uploading of evidence onto MIS system.
  • To assist and work alongside staff, participants, service users, visitors and volunteers during their time at IFA sites in their allocated duties
  • To personally develop skills, knowledge and expertise in gardening and community food growing, therapeutic horticulture, nutrition/food and health/mental health & wellbeing through working under supervision by the Growing Opportunities Project lead, Team Leaders and staff
  • To deliver, when required, activities linking to specific programme outcomes of our service level agreements, tenders and pilot projects (SLA’s, grant agreements, contracts)
  • To actively and professionally promote Ideal for All to potential new service users, health and local authorities, referral agencies and organisations through open days, presentations, display work and on and off site promotions.

    Key Tasks
  • To plan and deliver sessions and promotions to participants in order to support their skills development, employability and learning opportunities; whilst increasing confidence and supporting their health and wellbeing needs through horticulture to people of all ages and abilities in a variety of settings.
  • To operate within the company’s user led ethos and to pro-actively promote and encourage participation of disabled people of all ages.
  • To work to support people’s health and wellbeing using engaging, fun and educational initiatives utilising community focused delivery.
  • To ensure effective safeguarding and reporting for all participants whilst involved in the work of Ideal for All.
  • To provide cover for staff and to support with promotional events as requested.
  • To support in the development of health promoting activities based around exercise and healthy eating through the medium of horticulture.
  • To support the planning and delivery of accessible and safe programmes and practical courses, through on and off site workshops, day to day learning, open days and festivals.
  • To work closely and support project staff and other workers of Ideal for All, to ensure provision of an integrated service.
  • Keep up to date and accurate records of activities and involvements including client files, referral pathways, evaluation and evidence gathering, maintaining IFA’s electronic data systems and day-to-day administration duties.
  • Observe Health and Safety requirements in all areas of work and to be aware of the responsibilities under the Health & Safety at Work Act ensuring a safe environment is maintained for visitors, volunteers and staff.

Personal Assistant (NC)

Nature of the job role: 


Two Personal Assistant/Support Workers required for a 58 year old man living in the Cradley Heath area of Sandwell.  The gentleman is a diabetic and has other health illnesses related to the diabetes.
Ideally the service user would like to job share the role to include duties:-

Post 1
No personal care required however prompting is necessary from time to time.
To support, assist and motivate the service user to complete household duties   
Note taking/support at meetings
Support the service user to attend appointments i.e GP/ hospital appointments.
Making sure the service user is taking medication and eating properly due to diabetes.
To assist the service user in promoting independence by supporting them in the community. 
Must be able to drive

Post 2
Must be able to drive/tow vehicles
Must be fit and healthy as possible lifting is required i.e wooden pallets.  Service user is involved in voluntary work creating garden furniture
Interested in football as you will need to assist the service user in attending home/away games for a local semi pro football club as the service user is the team physio.  Support with carrying physio equipment

Personal duties: The client is in his late 30's, with a spinal cord and brain injury, he is totally reliant on his team for all aspects of care.

Personal Assistant (SB)

Nature of the job role: To be a personal assistant support worker to a boy with a learning disability who is none verbal. He is an outgoing and sociable person who enjoys the outdoors and physical activities.

Main Duties:

Social Duties: To provide full assistance and support in accessing social and physical activities within the local community.

Hours of work: An average of 5 hours per week. Typically used in blocks of a minimum of two hours per session. Times and days many vary so a degree of flexibility will be required

Qualifications: BSL Level 2 Skills and car driver with use of own vehicle preferred but not essential. The successful applicant will be required to complete an enhanced DBS check which would be paid for by the local authority.

Personal duties:  Ensure that the client is dressed appropriately before leaving the house.  Encourage client to wear incontinence pads if needed.  
Prepare breakfast for the client and encourage client to eat it. 
Accompany the client on outdoor activities such as shopping and travelling.

Monday to Friday 5 hours from 10:30am

Nature of the job role: Has a history of arthritis and back pain. She also has a diagnosis of dementia and needs support with all daily living tasks, unsteady on feet and requires the supervision of others, at significant risk of falls.
Main language is Punjabi, cannot speak or understand very much English.

Main Duties:

Personal duties: 
Unable to manage personal care independently, needs support with toileting day and night to ensure she is clean, dry and comfortable at all times.
Needs encouragement and guidance to eat and drink with support.
Assistance of others to change her pads and monitor skin integrity

Domestic duties: Prepare meals, drinks and snacks

Social Duties: Unable to access the local community independently and requires social interaction and cognitive stimulus.

Hours of work: 
Monday, Wednesday, Friday – 1 hour per day
A)    30 minutes at 5:00pm when she returns from day centre to change her clothes and personal hygiene/toileting and give meal.
B)    30 minutes at 8.30 pm to toilet, denture care, slight clothing change, fluid intake, general check

Tuesday, Thursday, Saturday, Sunday – 1½ hours per day
A) 30 minutes at lunch time to give lunch at 12:30 can be a little flexible. As long as it is the same time every day. Lunch, check clothing, Incontinence pad, and toilet, ensure she has dentures in for lunch, put into laundry basket any dirty clothes or incontinence pads in bin etc.
B) 30 minutes at 5:00pm when she returns from day centre to change her clothes and personal hygiene/toileting and give meal.
C) 30 minutes at 8.30pm to toilet, denture care, slight clothing change, fluid intake, general check.
Some change in the set times can be considered and adjusted to fit in better with carer if needs be only in communication with the main carer agreement and can be discussed further at interview

Client is a young girl who has cerebral palsy and multiple complex care needs and is fully dependant for all needs. She is unable to sit unaided, crawl or walk and has a specialist pushchair and home seating system.  Adult intervention and supervision is required at all times to facilitate play, feeding, changing, transfers etc. 
Client is a very happy smiley little girl who thrives on social interaction. 

Main Duties:

Initially to pick client up from school and return to the home to provide one to one support and care. (Working eventually towards client using school transport and carer to meet client at home). 

Personal duties:
Providing one to one support and care. 
Engaging with activities. 
Oral feeding. 
Changing if necessary.  

Domestic duties: 
Heating pre prepared food and preparing drinks. 

Social Duties: 
Accessing local area (park or going for a walk) after school in better weather periods. 
Accessing community in school holidays

Hours of work:

Main Duties:

Personal duties: Prompting and supporting with personal care at times and helping me maintain a routine where my basic needs are met. My condition is variable

Domestic duties: Assisting me with shopping and meal planning. Light domestic tasks to help me maintain and clean and safe environment. Food preparation and cooking as well as prompting to eat and drink  

Social Duties: Accompany me on social outings, accessing local amenities and medical appointments. Accompany me to Aquafit, swimming and walking therefore active person who can swim preferred 


Hours of work: 15 hours to be shared between 2 support workers / personal assistants

Nature of the job role:
Supporting a young girl living in Parkfield area of Wolverhampton with a diagnosis of Downs Syndrome, moderate hearing loss, visual impairment and severe learning disability.

Main Duties:

Personal duties: Assistance with toileting 

Domestic duties: Not applicable

Social Duties: Taking me out to access local amenities and social activities, if possible using your own car. On occasions you may be expected to keep me company in my home. 

Hours of work: Saturday 11.00am – 3.00pm

Qualifications: An Enhanced DBS (Disclosure and Barring Service) check will be required, funded by the Local Authority. Car driver preferred but not essential. Experience of working with Children with Downs Syndrome preferred but not essential.

Skills & Experience: You must be a friendly, understanding and positive person, with a good sense of humour and you must maintain strict confidentiality at all times. You should have a genuine interest in helping people on a one to one basis. An imaginative and friendly person who can suggest social outings of interest. Non-smoker preferred. Must be able to work reasonably flexible hours and be a good communicator. Previous experience of a supporting or caring role is preferred. It would be helpful if you have use of a car and a full driving license.

Working as part of a team. To assist with a variety of tasks which will support the employer to live an independent personal and social life.

Main Duties:

Personal duties: Nil

Domestic duties: Nil

Social Duties: To support the employer with social interaction and inclusion within the local community. The employer enjoys shopping, but would need support with enablement to do this as struggles with large crowds and noise. Whilst out, the PA will need to be aware of the surrounding environment as the employer has limited awareness of danger, especially when the location is noisy.

Hours of work: Currently 3 hours per week, but may go up in the future.

Qualifications: Level 2 NVQ/Diploma or similar in Social Care and First Aid would be beneficial, as would any training in cognitive or behavioural therapy/techniques.

Skills & Experience: Excellent communication skills are required to support the employer due to numerous sensory and auditory requirements that when accommodated for help to reduce anxiety. 

Previous experience of working with adults with Learning Difficulties/Autism would also be an advantage, as this role is very much to support the employer with everyday living and tasks.

The employer has high anxiety levels and the prospective PA would need to be very approachable, calm and have good coaching skills, with the ability to build a good rapport very quickly.

Nature of the job role: I am a disabled man, living with Cerebral Palsy in the Bushbury area of Wolverhampton with parents. I am wheelchair dependant and non-verbal, I communicate through a communication aid. 

Main Duties:

Personal duties: Washing, dressing, toileting and assisting with feeding; tube fed

Domestic duties: None

Social Duties: To escort me on social outings and accessing local amenities

Hours of work: Monday, Tuesday and Fridays 10.00am to 4.00pm

Qualifications: Full clean driver licence; Manual Handling

Skills & Experience: You should have a genuine interest in the provision of high quality care and should possess the motivation to develop trust and understanding within the role.
An understanding of computers would be preferable but not essential.
You should be hardworking, reliable, honest and be committed to providing the best care.

Main Duties:

Personal duties: Support with Personal Care, shower and dressing.

Domestic duties: Prepare Breakfast, heat ready prepared meals, make drinks, washing up and general household duties.

Social Duties: Nil at the moment, but this may change in the future.

Hours of work: Currently 14 hours per week, these hours may be split between 2 people or second employee to cover holidays.

Qualifications: Level 2 NVQ/Diploma or similar in Social Care and First Aid would be beneficial, as would any training in cognitive or behavioural therapy/techniques.

Skills & Experience: Experience/training in manual handling is required as the employer will need to be assisted with getting up and going to bed.

Previous experience of working with adults with limited mobility would be required, as this role is very much to support the employer with everyday living and tasks.

The employer has allergies and breathing difficulties, so the employee would need to be a non-smoker.

Any other reasonable duties that may be necessary.

Hours of work: 6 hrs per week (may possibly increase to an extra 1 hour morning call x 7 days per week shared between 2 PAs, but can be discussed at interview)

Qualifications: Level 2 NVQ/Diploma Health and Social Care or above

Skills & Experience:

The applicant needs to be a car driver with experience of handling a wheelchair and manual handling including the use of an overhead hoist.

Trained in the moving and Handling of People with limited mobility.

Accompany on various social and other activities, such as swimming, cinema / theatre visits, eating out, attending medical / therapy sessions and other appointments as required.

Personal duties: Bathing toileting dressing

Domestic duties: Snack preparation. Washing up, keeping bedroom and bathroom clean.
Social Duties: Accompanying on various social and other activities, such as swimming, cinema / theatre visits, eating out, attending medical / therapy sessions and other appointments as required.

Hours of work: Up to 24 hours per week to be worked flexibly; includes weekends and evenings

Qualifications:
Full clean driving licence. Manual Handling
Skills & Experience: Someone with a patient, caring and understanding manner with a good sense of humour. Some previous experience or relevant qualifications in area and working with wheelchair users would be preferable but not essential. Nature of the job role: Supporting a male with obstructive hydrocephalus which developed as a result of his underlying medical condition, Arnold Chiari, living in Stowheath, Wolverhampton
Surgery has reduced abilities greatly as he doesn't have any dexterity in his hands. He has reduced feeling in his hands which ads to his difficulties. Weakness to his left upper body for many years.
Left upper limb longstanding dense weakness secondary to brachial plexus injury. Right upper limb has learnt to compensate/adapt well to his weakness. Wears splints for bilateral upper limbs - left side cone splint and right resting splint. Bilateral palm shields prefers to wear posey on right hand as allows functional use of thumb.


Main Duties:

Personal duties: Assistance to wash and dress/undress daily. Cannot grip items. Assistance to bath a couple of times a week. Adjust clothing to feel comfortable. Someone to secure and maintain the property for me. I am unable to secure and maintain the safety in the home as physically I cannot grip items or push pull. Opening a door or lock is a major problem.

Domestic duties: To ensure he eats a well-balanced meal and have assistance with eating and drinking when required. Assistance to ensure shopping is undertaken

Social Duties: To access the community / gym

Hours of work: 26 hours per week
Monday – Saturday
9:00am – 10:00am 1:00pm – 2:00pm 6:00pm – 7:00pm
Plus 2 hours to support with household, food shopping when required
6 hours as required to access the community

Main Duties: To support an elderly lady of Black Caribbean background. Living in Whitmore Reans, Wolverhampton.  She has Dementia, also has some confusion and disorientation.


Personal duties: Encourage and support to take a shower, to re offer if needed, support with incontinence and skin integrity, to encourage breakfast, sit with her to encourage this, encourage her to take medication

Domestic duties: Need to encourage food, fluids, leave snacks and encourage personal care. Change bed linen when required

Social Duties: Needs someone who is sensitive to West Indian culture

Hours of work: 
9am for 1 hour (for personal care, to offer medication and give fluids and breakfast, leave a snack, making bed, changing bed linen if appropriate, check bins.  Encourage an activity. Check that the Satnav device is in place on her person)
1pm for 30 minutes (for encouragement of eating the hot meal delivery, leaving fluids and snack for later)
6pm for 1 hour (for encouraging her to eat and take fluids, to change for bed and check all is locked and safe)
